ABSTRACT

El síndrome de Eagle está caracterizado por una elongación o una curvatura medial excesiva de la apófisis estiloides o por una calcificación del ligamento estilohioideo que puede provocar dolor cervicofacial o síntomas neurológicos por la compresión de los vasos o nervios del cuello. El tratamiento más eficaz es el quirúrgico y consiste en la resección de la apófisis estiloides; puede ser realizado por vía externa o mediante un abordaje transoral. Se describe el caso clínico de un paciente con síndrome de Eagle que fue tratado con éxito mediante un abordaje transoral, sin amigdalectomía y con asistencia de endoscopios. (AU)


Eagle syndrome is characterized by an elongation or excessive medial curvature of the styloid process or calcification of the stylohyoid ligament that can cause cervicofacial pain or neurological symptoms due to compression of the vessels or nerves of the neck. The most effective treatment is surgical and consists of resection of the styloid process, it can be performed by externally or through a transoral approach.The clinical case of a patient with Eagle syndrome who was successfully treated by a transoral approach, without tonsillectomy and with the assistance of endoscopes, is described. ABSTRACT

Introduction: The anatomy of the temporal bone is complex due to the large number of structures and functions grouped in this small bone space, which do not exist in any other region in the human body. With the difficulty of obtaining anatomical parts and the increasing number of ear, nose and throat (ENT) doctors, there was a need to create alternatives as real as possible for training otologic surgeons. Objective: Developing a technique to produce temporal bone models that allow them to maintain the external and internal anatomical features faithful to the natural bone. Methods: For this study, we used a computed tomography (CT) scan of the temporal bones of a 30-year-old male patient, with no structural morphological changes or any other pathology detected in the examination, which was later sent to a 3D printer in order to produce a temporal bone biomodel. Results: After dissection, the lead author evaluated the plasticity of the part and its similarity in drilling a natural bone as grade "4" on a scale of 0 to 5, in which 5 is the closest to the natural bone and 0 the farthest from the natural bone. All structures proposed in the method were found with the proposed color. Conclusion: It is concluded that it is feasible to use biomodels in surgical training of specialist doctors. After dissection of the bone biomodel, it was possible to find the anatomical structures proposed, and to reproduce the surgical approaches most used in surgical practice and training implants (AU)

RESUMEN: La condromatosis sinovial (CS), es una lesión benigna poco frecuente y de clínica bastante inespecífica. Suele afectar articulaciones de huesos largos como la rodilla, el codo y la cadera, presentándose generalmente de manera unilateral. Se cree que solo un 3 % de los casos de CS afecta la articulación temporomandibular. Esta condición se caracteriza por ser un trastorno metaplásico del tejido conectivo sinovial que suele manifestarse con la formación de pequeños y múltiples nódulos de cartílago que posteriormente pueden desprenderse, calcificarse y formar cuerpos libres dentro del espacio articular. Presentamos el caso de una mujer de 55 años con condromatosis sinovial de la articulación temporomandibular, tratada desde hace 3 años bajo el diagnóstico de desórdenes temporomandibulares. A pesar de ser considerada una lesión de tipo benigna, esta puede llegar a ser localmente agresiva, extendiéndose como en nuestro reporte hacia la fosa craneal media, adelgazando parte del hueso temporal.


ABSTRACT: Synovial chondromatosis (CS) is a benign lesion that is rare and clinically quite nonspecific. It usually affects the joints of long bones such as the knee, elbow and hip, usually occurring unilaterally. It is believed that in only 3 % of cases of CS the temporomandibular joint. This is a condition its characterized by being a metaplastic synovial connective tissue that manifests itself with the formation of small and multiple cartridges that detach, calcify and form free bodies within the joint space. We present the case of a 55-year-old woman with synovial chondromatosis of the temporomandibular joint, treated for 3 years under the diagnosis of temporomandibular disorders. Despite being considered a benign lesion, this can become locally aggressive, extending as in our report to the cranial fossa, thinning part of the temporal bone.

ABSTRACT

Abstract Introduction The literature shows that there are anatomical changes on the temporal bone anatomy during the first four years of life in children. Therefore, we decided to evaluate the temporal bone anatomy regarding the cochlear implant surgery in stillbirths between 32 and 40 weeks of gestational age using computed tomography to simulate the trajectory of the drill to the scala timpani avoiding vital structures. Objectives To measure the distances of the simulated trajectory to the facial recess, cochlea, ossicular chain and tympanic membrane, while performing the minimally invasive cochlear implant technique, using the Improvise imaging software (Vanderbilt University, Nashville, TN, US). Methods An experimental study with 9 stillbirth specimens, with gestational ages ranging between 32 and 40 weeks, undergoing tomographic evaluation with individualization and reconstruction of the labyrinth, facial nerve, ossicular chain, tympanic membrane and cochlea followed by drill path definition to the scala tympani. Improvise was used for the computed tomography (CT) evaluation and for the reconstruction of the structures and trajectory of the drill. Results Range of the distance of the trajectory to the facial nerve: 0.58 to 1.71mm. to the ossicular chain: 0.38 to 1.49 mm; to the tympanic membrane: 0.85 to 1.96 mm; total range of the distance of the trajectory: 5.92 to 12.65 mm. Conclusion The measurements of the relationship between the drill and the anatomical structures of the middle ear and the simulation of the trajectory showed that the middle ear cavity at 32 weeks was big enough for surgical procedures such as cochlear implants. Although cochlear implantation at birth is not an indication yet, this study shows that the technique may be an option in the future.

ABSTRACT Purpose To study the intraoperative findings in case of early and delayed decompression of facial nerve paralysis and compare their results. Methods Retrospective data analysis of 23 cases of longitudinal temporal bone fracture with House-Brackmann grade V and VI facial nerve paralysis. All cases were thoroughly evaluated and underwent facial nerve decompression through the transmastoid approach. All cases were under regular follow-up till the date of manuscript submission. Results Clinical improvement of the facial nerve function was observed for early vs. delayed facial nerve decompression. In the early decompression group, facial nerve function improved to grade II in eight cases (80%) and grade III in two cases (20%), whereas in the delayed decompression group it improved to grade II in one case (7.70%), grade III in four cases (30.76%), grade IV in seven cases (53.84%), and grade V in one case (7.70%). Conclusions Early decompression of facial nerve provides better results than delayed decompression because it enables early expansion of the nerve.

RESUMEN Introducción: El sinus tympani (ST) es una de las áreas que más presenta colesteatoma residual. Recientemente se ha clasificado en 3 tipos de acuerdo a su morfología. Objetivos: Determinar el tipo de ST en los pacientes sometidos a cirugía de colesteatoma y analizar su impacto como factor de recidiva. Material y método: Revisión de fichas clínicas de pacientes sometidos a cirugía de colesteatoma entre los años 2004 y 2015 en el Hospital Regional de Concepción. Análisis de la tomografía axial computarizada (TAC) preoperatoria y posterior evaluación clínica de los pacientes operados mediante mastoidectomía canal wall down (CWD). Resultados: En el periodo descrito se operaron 271 oídos. El 60% de los casos analizados presentó ST tipo A y 40% ST tipo B. Se identificaron 12 casos de recidiva, 3 ST tipo B y 9 ST tipo A, sin diferencia estadísticamente significativa entre ambos. Discusión: Distinto a lo reportado en la literatura el tipo de ST más frecuente en nuestro estudio fue el tipo A, lo que podría corresponder a una variable étnica. Conclusión: El estudio preoperatorio con TAC es una herramienta útil para evaluar el tipo y compromiso del ST. Las diferencias anatómicas entre ST tipo A y B parece no ser un factor determinante de recidiva en mastoidectomías CWD.


ABSTRACT Introduction: Sinus tympani (ST) is one of the areas with the most residual cholesteatoma. Recently it has been classified in 3 types according to its morphology. Aim: To determine the type of ST in patients undergoing cholesteatoma surgery and to analyze its impact as a relapse factor. Material and method: Review of clinical files of patients submitted to cholesteatoma surgery between 2004 and 2015 at the Regional Hospital of Concepción. Preoperative computed axial tomography (CT) analysis and subsequent clinical evaluation of patients operated by canal wall down mastoidectomy (CWD). Results: In the described period 271 ears were operated. 60% of the cases analyzed had ST type A and 40% ST type B. Twelve cases of relapse were identified, 3 ST type B and 9 ST type A, with no statistically significant difference between the two. Discussion: Unlike to what is reported in the literature, the most common ST type in our study was type A, which could correspond to an ethnic variable. Conclusion: The preoperative study with CT is a useful tool to evaluate the type and commitment of ST. The anatomical differences between ST type A and B seems not to be a determinant factor of relapse in CWD mastoidectomies.

ABSTRACT

Temporomandibular joint ankylosis (TMA) is a highly distressing condition in which the temporomandibular joint (TMJ) is replaced by scar tissue. Most frequently reported complications after surgical treatment are limited mouth opening and re-ankylosis. Reankylosis happens due to inadequate bone removal, lack of sufficient interpositional material, fibrous tissue adhesions and elongation of coronoid process and regrowth of bone in the sigmoid notch area. In gap arthroplasty treatment chances to recurrence is 53% than interpositional arthroplasy. We treated a case of right sided TMJ reankylosis by interpositional arthroplasty with temporomyofacial flap and physiotherapy was started 3 days after surgery and maintained for 6 months. In 2 years follow-up, no signs of recurrence and maximum mouth opening 45 mm were observed. The success in preventing reankylosis after TMJ interpositional arthroplasty with temporomyofacial flap is relatively better than gap arthroplasty alone.

Introdução: A paralisia facial periférica caracteriza-se pela interrupção, definitiva ou temporária, do funcionamento da musculatura facial. Decorre de lesão ou mau funcionamento das fibras do nervo facial. É uma enfermidade que causa enorme impacto estético e funcional. O topodiagnóstico e o tratamento da paralisia facial periférica pode requer abordagem médica e fonoaudiológica; ser exclusivamente medicamentoso ou associado à terapia de reabilitação, ou ainda, medicamentoso e cirúrgico, seguido da reabilitação da mímica facial. A via FCM tem sido usada para a descompressão do NF quando a audição precisa ser preservada. Esse acesso pode ser realizado de forma isolada, ou combinado à via transmastóidea. Objetivo: Descrever uma técnica inovadora para a descompressão do nervo facial via fossa craniana média que permite a exposição direta dos segmentos labiríntico e timpânico do nervo facial, com a preservação da função da orelha interna. Métodos: Vinte cabeças extraídas de cadáveres adultos de ambos os gêneros, sem sinais de malformação, traumatismo, doença ou manipulação cirúrgica prévia foram usados neste estudo. Os pontos de referência utilizados foram a artéria meníngea média, o nevo petroso superficial maior, a eminência arqueada, o seio petroso superior, e o plano meatal seguido no ápice petroso a partir da sua porção mais anterior e medial. Foi feita a dissecação do plano meatal, com visualização do meato acústico interno, seguido no ápice petroso a partir da sua porção mais anterior e medial até a região do gânglio geniculado. Foi aberto o tégmen timpânico e identificada a porção timpânica do nervo facial. A dissecação seguiu no sentido retrógrado da porção timpânica do nervo facial em direção ao gânglio geniculado, até a sua porção labiríntica. A aracnoide do meato acústico interno era aberta, e depois de identificado o nervo facial, a bainha deste nervo era aberta em extensão exposta. Background: Peripheral facial palsy is characterized by the permanent or temporary interruption of the functioning of the facial muscles. The middle cranial fossa (MCF) approach has been used for the decompression of the facial nerve (FN) when hearing needs to be preserved. In this work, we describe an innovative technique for the decompression of the FN through the MCF approach that allows the direct exposure of the labyrinthine and entire tympanic segment of the FN, with the preservation of inner ear function. Methods: Twenty cadavers heads were used in this study. The reference landmarks used were the middle meningeal artery, the greater superficial petrosal nerve, the arcuate eminence, the inferior petrosal sinus and the meatal plane following the petrous apex from its most anterior and medial portion. Results: The tympanic segment of the FN presented, on average, a total length of 11 ± 0.67mm to the right, and 11.5 ± 0.60mm to the left. The longitudinal lengths of bone window in the tegmen tympani were 16.8±1.67mm to the right, and 16.8 ± 1.20mm to the left. The cross-sectional lengths of the bone window in the tegmen tympani were 5.5 ± 1.20mm and 5.0±1.75mm to the right and left sides, respectively. The average value of elliptical area formed by the longitudinal and transversal lengths of the bone window made in the tegmen tympani were 72.5 ± 22.5mm2 to the right, and 65.9 ± 30.3mm2 to the left. Conclusion: The proposed technique can be used for the surgical decompression of the tympanic, labyrinthine and meatal segments of the FN through the MCF, without imposing a risk to hearing, in addition to reducing the surgical time and the risk to patients.

ABSTRACT

El neumoencéfalo espontáneo de origen otológico es una patología potencialmente mortal, infrecuente y escasamente reportada en la literatura. Resulta interesante conocer su presentación clínica y manejo. Para el diagnóstico es importante tener la sospecha clínica y el estudio de imágenes con tomografía computarizada (TC) habitualmente muestra la dehiscencia en el hueso temporal. El tratamiento debe tener un enfoque multidisciplinario. La decisión de tratamiento conservador y quirúrgico va estar determinado por la gravedad del paciente y características del defecto.


Pneumocephalus otic spontaneous origin is a potentially fatal disease, infrequent and rarely reported in the literature. It is interesting to know its clinical presentation and management. To diagnose it is important to clinical suspicion and imaging study with CT usually shows dehiscence of the temporal bone. Treatment should be a multidisciplinary approach. The decision of conservative and surgical treatment will be determined by the severity of the patient and defect characteristics.

INTRODUÇÃO: A ampliação dos critérios de indicação para cirurgia do implante coclear e os benefícios da preservação da audição residual no pósoperatório estimularam o desenvolvimento de técnicas cirúrgicas atraumáticas. Minimizar os traumas intracocleares durante a inserção do feixe de eletrodos do implante coclear é um passo fundamental para este intuito. O objetivo deste trabalho é avaliar se o trauma intracoclear é diferente quando o feixe de eletrodos do implante coclear é inserido através do quadrante anterossuperior ou anteroinferior da membrana da janela redonda. MÉTODOS: Vinte e cinco ossos temporais frescos de cadáveres humanos foram submetidos à timpanomastoidectomia padrão. Após exposição adequada da membrana da janela redonda, em metade dos ossos o feixe de eletrodos do implante coclear foi inserido via quadrante anterossuperior da membrana da janela redonda, e na outra metade via quadrante anteroinferior. Os ossos temporais foram desidratados e embebidos em epóxi, com o feixe de eletrodos in situ. As peças foram serialmente polidas, tingidas e visualizadas por meio de estereomicroscópio para avaliar o trauma intracoclear causado pela inserção do feixe de eletrodos. As imagens foram fotografadas. RESULTADOS: Em treze ossos temporais o feixe de eletrodos do implante coclear foi inserido via quadrante anterossuperior da membrana da janela redonda, e em doze ossos via quadrante anteroinferior. Obteve-se 372 superfícies. As análises histológicas revelaram diferentes graus de traumas às estruturas intracocleares. Os resultados mostraram que a inserção do feixe de eletrodos via quadrante anterossuperior ou anteroinferior acarretam a mesma frequência de trauma intracoclear. CONCLUSÕES: A presença de trauma intracoclear e a severidade dos traumas ocorridos no tocante à inserção do feixe de eletrodos de implante coclear pelo quadrante anterossuperior e anteroinferior da membrana da janela redonda não apresentou diferença estatisticamente significativa...


INTRODUCTION: The expansion of the indication criteria for cochlear implant surgery and the benefits of preserving residual hearing postoperatively have stimulated the development of atraumatic surgeries. Minimizing the intracochlear traumas during the electrodes insertion is a critical step for this aim. The objective of this study is to assess whether there is a difference in intracochlear trauma when the cochlear implant electrode array is inserted through the anterior-superior or anterior-inferior quadrants of the round window membrane. METHODS: Twenty-five fresh human temporal bones were submitted to standard tympanomastoidectomy. After adequate exposure of the round window membrane, in half of the bones the cochlear implant electrode array was inserted via anterior-superior quadrant of round window membrane and in the other half via anterior-inferior quadrant. The temporal bones were dehydrated and embedded in epoxy with the electrodes array in situ. The specimens were serially polished, stained and viewed through a stereomicroscope to assess the intracochlear trauma caused by insertion of the electrode array. Resulting images were documented. RESULTS: In thirteen temporal bones the cochlear implant electrode array was inserted via anterior-superior quadrant of round window membrane and in twelve bones via anterior-inferior quadrant. Three hundred and seventy two surfaces were obtained. Histological examinations revealed varying degrees of damage to the intracohlear structures. The results showed that the insertion of the electrode array via anterior-superior or anterior-inferior quadrant lead to the same frequency of intracochlear trauma. CONCLUSIONS: The presence of intracochlear trauma and severity of traumas regarding the insertion of cochlear implant electrode array via anterior-superior and via anterior-inferior quadrant of the round window membrane showed no statistically significant difference. Introdução: A técnica clássica para o implante coclear é realizada por meio de mastoidectomia seguida de timpanotomia posterior. O acesso pela fossa craniana média provou ser uma alternativa valiosa, embora seja usada para o implante coclear, ainda sem normatização. Objetivo: Descrever um novo acesso pela fossa craniana média que expõe o giro basal da cóclea para o implante coclear. Métodos: Estudo anatômico de ossos temporais. Foram dissecados 50 ossos temporais. A cocleostomia foi realizada mediante um acesso via fossa craniana média, na porção mais superficial do giro basal da cóclea, usando o plano meatal e seio petroso superior como as principais referências anatômicas. Foi determinada a distância entre os pontos de referência, o ângulo entre o plano meatal e a cocleostomia, e a distância entre esta estrutura e a janela redonda. Foi realizada tomografia computadorizada em 5 dos ossos temporais utilizados neste estudo. Resultados: Em todos os 50 ossos temporais, apenas a porção mais superficial do giro basal da cóclea foi aberta e tanto as escalas timpânica como a vestibular foram visualizadas. As distâncias médias ± DP, menores e maiores, entre a cocleostomia e o plano meatal foram estimadas em 2,48±0,88mm e 3,11±0,86mm, respectivamente. A distância média da cocleostomia até a janela redonda foi de 8,38±1,96mm, e daquela até o seio petroso superior 9,19±1,59mm. As distâncias médias, menores e maiores, entre a cocleostomia e o eixo longo do plano meatal a partir da sua porção mais proximal foram estimadas em 6,63±1,38mm e 8,2±1,43mm, respectivamente. O valor médio do ângulo entre a cocleostomia e o plano meatal foi igual a 22,54±7,400. As tomografias computadorizadas demonstraram a inserção do feixe de eletrodos por meio do giro basal da cóclea até o seu ápice em todas as peças submetidas a este exame. Introduction: The classic technique for cochlear implantation uses mastoidectomy followed by posterior tympanotomy. The middle cranial fossa approach has proved to be a valuable alternative for cochlear implantation, although the standardization of this technique is still needed. Objectives: To describe a novel approach through the middle cranial fossa for exposing the cochlear basal turn for cochlear implantation. Materials And Methods: Anatomical temporal bone study. Fifty temporal bones were dissected. A cochleostomy was performed via a middle fossa approach on the most superficial part of the cochlear basal turn, using the meatal plane and superior petrous sinus as the main landmarks. The distance between the landmarks, the angle between the cochleostomy and the meatal plane, and the distance between this structure and the round window were measured. A computed tomography was performed on 5 of the studied temporal bones. Results: In all 50 temporal bones, only the superficial portion of the cochlear basal turn was uncovered. The cochlear exposure allowed both the scala tympani and vestibule to be exposed. The mean ± SD minor and major distances between the cochleostomy and the meatal plane were estimated to be 2.48±0.88mm and 3.11±0.86mm, respectively. The mean distance from the cochleostomy to the round window was 8.38±1.96mm, and that to the superior petrosal sinus was 9.19±1.59mm. The mean minor and major distances between the cochleostomy and the long axis of the meatal plane from its most proximal portion were estimated to be 6.63±1.38mm and 8.29±1.43mm, respectively. The mean angle between the cochleostomy and the meatal plane was 22.54±7.400. The computed tomography of all 5 temporal bones demonstrated the insertion of the implant array from the cochlear basal turn towards its apex. ABSTRACT

PURPOSE: Initial study of the pig`s temporal bone anatomy in order to enable a new experimental model in ear surgery. METHODS: Dissection of five temporal bones of Sus scrofa pigs obtained from UNIFESP - Surgical Skills Laboratory, removed with hole saw to avoid any injury and stored in formaldehyde 10% for better conservation. The microdissection in all five temporal bone had the following steps: inspection of the outer part, external canal and tympanic membrane microscopy, mastoidectomy, removal of external ear canal and tympanic membrane, inspection of ossicular chain and middle ear. RESULTS: Anatomically it is located at the same position than in humans. Some landmarks usually found in humans are missing. The tympanic membrane of the pig showed to be very similar to the human, separating the external and the middle ear. The middle ear`s appearance is very similar than in humans. The ossicular chain is almost exactly the same, as well as the facial nerve, showing the same relationship with the lateral semicircular canal. CONCLUSION: The temporal bone of the pigs can be used as an alternative for training in ear surgery, especially due the facility to find it and its similarity with temporal bone of the humans. .

ABSTRACT

A técnica clássica para o implante coclear é realizada através de mastoidectomia e timpanotomia posterior. A abordagem pela fossa craniana média provou ser uma alternativa valiosa, embora venha sendo usada para o implante coclear apenas esporadicamente e sem normatização. OBJETIVO: Descrever uma nova abordagem para expor o giro basal da cóclea para o implante coclear através da fossa craniana média. MÉTODO: Cinquenta ossos temporais foram dissecados. A cocleostomia foi realizada através de uma abordagem via fossa craniana média, na parte mais superficial do giro basal da cóclea, usando o plano meatal e seio petroso superior como pontos de reparo. A parede lateral do meato acústico interno foi dissecada após o broqueamento e esqueletização do ápice petroso. A parede dissecada do meato acústico interno foi acompanhada longitudinalmente até a cocleostomia. Design: Estudo anatômico de osso temporal. RESULTADOS: Em todos os ossos temporais, apenas a parte superficial do giro basal da cóclea foi aberta. A exposição do giro basal da cóclea permitiu que as escalas timpânica e vestibular fossem visualizadas. Assim, não houve dificuldade na inserção do feixe de eletrodos através da escala timpânica. CONCLUSÃO: A técnica proposta é simples e permite exposição suficiente do giro basal da cóclea.


The classic approach for cochlear implant surgery includes mastoidectomy and posterior tympanotomy. The middle cranial fossa approach is a proven alternative, but it has been used only sporadically and inconsistently in cochlear implantation. OBJECTIVE: To describe a new approach to expose the basal turn of the cochlea in cochlear implant surgery through the middle cranial fossa. METHOD: Fifty temporal bones were dissected in this anatomic study of the temporal bone. Cochleostomies were performed through the middle cranial fossa approach in the most superficial portion of the basal turn of the cochlea, using the meatal plane and the superior petrous sinus as landmarks. The lateral wall of the internal acoustic canal was dissected after the petrous apex had been drilled and stripped. The dissected wall of the inner acoustic canal was followed longitudinally to the cochleostomy. RESULTS: Only the superficial portion of the basal turn of the cochlea was opened in the fifty temporal bones included in this study. The exposure of the basal turn of the cochlea allowed the visualization of the scala tympani and the scala vestibuli, which enabled the array to be easily inserted through the scala tympani. CONCLUSION: The proposed approach is simple to use and provides sufficient exposure of the basal turn of the cochlea.

ABSTRACT

This review intended to describe in a didactic and practical manner the frontotemporosphenoidal craniotomy, which is usually known as pterional craniotomy and constitute the cranial approach mostly utilized in the modern neurosurgery. This is, then, basically a descriptive text, divided according to the main stages involved in this procedure, and describes with details how the authors currently perform this craniotomy.


A presente revisão visou descrever de forma didática e prática a realização da craniotomia frontotemporoesfenoidal, usualmente denominada pterional, que constitui a craniotomia mais utilizada na prática neurocirúrgica atual. Trata-se, portanto, de um texto fundamentalmente descritivo, dividido conforme as principais etapas envolvidas na realização desse procedimento, que mostra com detalhes a técnica utilizada atualmente pelo presente grupo de autores.

É difícil encontrar ossos temporais humanos para o ensino de cirurgia otológica. Ossos temporais de ovelhas podem representar uma possível alternativa. MATERIAIS E MÉTODOS: Os ossos temporais de ovelhas foram dissecados em um programa convencional de dissecação de osso temporal no laboratório. Incluímos mastoidectomia, abordagens endaurais, mas também analisamos a aparência externa, o meato acústico externo e o hipotímpano. Algumas etapas são diferentes das preparações de ossos humanos. Os resultados morfométricos foram comparados à anatomia conhecida de humanos para verificar se o osso temporal de ovelhas seria utilizável para o ensino de cirurgia otológica. RESULTADOS: O osso temporal de ovelhas parece menor do que o humano. Encontramos uma área bolhosa se estendendo ao hipotímpano abaixo do meato acústico externo. A membrana timpânica é muito semelhante à humana. O meato acústico externo é menor e mais curto. A cadeia ossicular exibe analogias para com a humana. DISCUSSÃO: Esse estudo mostra que especificamente o ouvido médio, a membrana timpânica e o conduto auditivo externo são morfologicamente semelhantes às suas contrapartidas encontradas nos ossos temporais humanos. A ovelha parece ser um modelo viável para o ensino da anatomia do ouvido. A menor escala de algumas estruturas, especialmente dos componentes externos do osso temporal representa uma desvantagem. CONCLUSÕES: A ovelha parece representar uma alternativa viável no ensino de cirurgia otológica.


Human temporal bones in teaching ear surgery are rare. The lamb's temporal bone might be a possible alternative. MATERIAL AND METHODS: Temporal bones of the lamb were dissected with a typical temporal bone lab drilling program. We included a mastoidectomy, endaural approaches, but also analyzed the outer appearance, the external ear canal and the hypotympanon. Some steps differed from preparation done in humans. The morphometric results were compared to the known anatomy of human in order to verify the lambs' temporal bone for suitability in otosurgic training. RESULTS: The lambs' temporal bone appears smaller than the human one. We found a bullous extended hypotympanon located under the external ear canal. The tympanic membrane is very similar to the human one. The external ear canal is smaller and shorter. The ossicular chain shows analogies to human one. DISCUSSION: This study shows, that especially the middle ear, the tympanic membrane and the external ear canal are morphologically equal to the structures found in human temporal bones. The lamb seems feasible for teaching the anatomy of the ear. The smaller scales of some structures, especially the outer components of the temporal bone are a disadvantage. CONCLUSIONS: The lamb seems to be an alternative in teaching ear surgery.

ABSTRACT

Elongated styloid process causing Eagle's syndrome is a rare clinical entity and the diagnosis is often difficult as result of its vague symptomatology. However, palpation of tonsillar fossa with radiological demonstration of the elongated styloid process confirms the diagnosis. This is a report of a retrospective study conducted at the KVG Medical College, Hospital, Sullia, Karnataka, India where 15 patients who were surgically treated for Eagle's syndrome in the ENT department were retrospectively studied. Fourteen patients became symptom free after surgery within three months of follow up. Tonsillo-styloidectomy is the treatment of choice for Eagle's syndrome with a high success rate

A dissecção em ossos temporais tem papel fundamental na formação de cirurgiões e sua obtenção esbarra em diversas dificuldades. OBJETIVO: Desenvolver uma réplica sintética do osso temporal para treinamento de dissecção. FORMA DE ESTUDO: Experimental. MATERIAIS E MÉTODOS: Foi desenvolvida uma réplica de resina acrílica termopolimerizante através de técnicas de moldagem com silicone. Para a avaliação do método, foram selecionados cinco cirurgiões otológicos para dissecção do modelo em laboratório de cirurgia experimental. Foi preenchido questionário, levando em consideração a aparência externa, a simulação de procedimentos (colocação de tubo de ventilação, mastoidectomia, descompressão do nervo facial e acesso translabiríntico ao meato acústico interno) e a impressão final. RESULTADOS: A avaliação apontou satisfação na utilização do modelo (80 por cento), sendo mais evidente no que se referiu à dissecção do segmento mastoide do nervo facial e ao acesso translabiríntico ao meato acústico interno. A colocação de tubo de ventilação foi razoável para 60 por cento e satisfatória para 40 por cento deles. A mastoidectomia foi totalmente satisfatória para 40 por cento. CONCLUSÃO: A dissecção neste simulador otológico não substitui o treinamento em ossos temporais de cadáveres, porém, dada a crescente dificuldade na obtenção destes, o desenvolvimento de novas ferramentas de ensino deve ser encorajado para o contínuo aprimoramento de cirurgiões.


Temporal bone dissection plays an important role in the training of surgeons; however, they are difficult to obtain. AIM: To develop a synthetic replica of the temporal bone for dissection training. STUDY DESIGN: Experimental. MATERIALS AND METHODS: An acrylic synthetic resin replica was obtained from a human temporal bone. For the evaluation of the method, we selected five ear surgeons to dissect the model in a laboratory of experimental surgery. A questionnaire was filled, assessing external appearance, the simulation of procedures (placement of ventilation tube, mastoidectomy, decompression of the facial nerve and translabyrinthine access to the internal auditory canal) and their final conclusion. RESULTS: The evaluation indicated satisfaction in using the model (80 percent), being more evident concerning the dissection of the mastoid segment of the facial nerve and translabyrinthine access to the internal auditory canal. The placement of a ventilation tube was reasonable for 60 percent and satisfactory for 40 percent of them. Mastoidectomy was satisfactory for 60 percent and fully satisfactory for 40 percent. CONCLUSION: Dissection in this simulator does not replace otologic training in cadaveric temporal bones. However, given the increasing difficulty in obtaining the latter, the development of new teaching tools should be encouraged to continuously improve surgeons.

A ossificação da cóclea, decorrente principalmente de meningite, impede a inserção completa do implante coclear convencional. Os implantes com 2 feixes de eletrodos mais curtos do que o convencional foram desenvolvidos especialmente para cócleas ossificadas. Porém, durante essa cirurgia há um grande risco de lesão da artéria carótida interna (ACI). Portanto, a medida da profundidade das cocleostomias para inserir os dois feixes de eletrodos aumentaria a segurança desse procedimento. OBJETIVOS: 1) Obter as distâncias entre as cocleostomias e a ACI em ossos temporais de cadáver; 2) Confeccionar instrumento que possa ser usado na cirurgia de implante coclear com 2 feixes de eletrodos. FORMA DE ESTUDO: Experimental prospectivo. MATERIAL E MÉTODO: Em 21 ossos de cadáveres foi realizada: 1) mastoidectomia cavidade aberta; 2) cocleostomias nos giros basal e médio da cóclea; 3) identificação da ACI; 4) medida da distância entre as cocleostomias e a artéria. RESULTADOS: A medida média ± desvio padrão obtida para o túnel superior foi 8,2 ± 1,1mm e para o túnel inferior foi 8,1± 1,3mm. A menor distância encontrada foi 6,5mm para o túnel superior e 6,0mm para o túnel inferior. CONCLUSÃO: Apesar dos parâmetros calculados concluímos que a melhor medida para ser considerada na confecção do instrumento cirúrgico serão as mínimas medidas obtidas em cada um dos giros cocleares, pois é a maneira mais segura para evitar a lesão da ACI, que pode ser fatal.


Cochlear ossification, mainly secondary to meningitis, prevents the complete conventional cochlear implant insertion. Implants with two electrode bundles shorter than the conventional ones were specifically developed for ossified cochleas. However, during surgery there is a high risk of damaging the internal carotid artery (ICA). Therefore, measuring cochleostomy depth in order to insert the two electrode bundles would greatly increase the procedure's safety. AIMS: 1) Find the distances between cochleostomies and ICA in cadaver temporal bones. 2) Design an instrument that can be used in cochlear implant surgery to introduce an implant with two bundles of electrodes. STUDY DESIGN: Experimental prospective. MATERIALS AND METHODS: In 21 temporal bones from cadavers we performed: 1) canal wall down mastoidectomy; 2) cochleostomy in the cochlear basal and middle turns; 3) ICA identification; 4) Length determination between the cochleostomies and the artery. RESULTS: the average distance ± standard deviation obtained for the upper tunnel was of 8.2 ± 1.1 mm and for the lower tunnel it was of 8.1± 1.3 mm. The shortest distance found was of 6.5 mm for the upper tunnel and 6.0 mm for the lower tunnel. CONCLUSION: Despite the values calculated, we concluded that the best value to be considered in creating a surgical instrument are the minimum lengths obtained for each one of the cochlear turns, because this is the safest way to avoid damaging the ICA, that can be fatal.

ABSTRACT

PURPOSE: To evaluate tympanic bulla healing after experimental lateral osteotomy in cats. METHODS: Twenty adult cats were submitted to unilateral lateral bulla osteotomy and divided into two groups: cats of B1 group (n=10) were euthanized at 8 weeks and cats of B2 group (n=10), at 16 weeks postoperative. RESULTS: Oblique lateral radiographs taken immediately postoperative showed interruption in the contour of the external acoustic meatus of the operated bullae in all cats of both groups (McNemar test: p=0.0010*). This feature was still observed in the radiographs taken after 8 and 16 weeks postoperative (McNemar test: B1 p=0.0020*; B2 p=0.0312*). Macroscopic examination showed that the operated bullae were similar to the normal ones, with preservation of the tympanic cavity. Connective tissue at the osteotomy site was significantly found in the operated bullae in both groups (McNemar test: B1 p=0.0020*; B2 p=0.0010*). The length of connective tissue at the osteotomy site was measured by histomorphometry. There was no statistically significant difference between the values of B1 group and B2 group (Mann-Whitney test: p=0.0524). CONCLUSIONS: Experimental lateral osteotomy did not alter significantly the tympanic bulla conformation and complete regeneration of the tympanic bulla frequently did not occur before 16 weeks of postoperative period.


OBJETIVO: Avaliar a morfologia da bulla tympanica de gatos após osteotomia lateral unilateral. MÉTODOS: Foram utilizados 20 gatos distribuídos em dois grupos de 10 animais cada, de acordo com o período de observação: B1 (8 semanas) e B2 (16 semanas). RESULTADOS: Nas radiografias em projeção lateral oblíqua realizadas no pós-operatório imediato observou-se a interrupção do meato acústico externo da bulla tympanica operada de todos os gatos de ambos os grupos (McNemar, p=0,0010*). Esta característica ainda pode ser observada nas radiografias realizadas 8 ou 16 semanas após a cirurgia (McNemar: B1 p=0.0020*; B2 p=0.0312*). Os exames macroscópicos revelaram que a bulla tympanica operada apresentava conformação semelhante a da bulla tympanica normal, com preservação da cavidade timpânica. Na maioria das bullae tympanicae operadas observou-se a presença de tecido conjuntivo na área de osteotomia. O resultado do teste de McNemar foi significante em ambos os grupos (B1 p=0,0020*; B2, p=0,0010*). A histomorfometria mediu a extensão de tecido conjuntivo presente no local da osteotomia. Não houve diferença estatisticamente significante entre os dois grupos (Mann-Whitney, p=0,0524). CONCLUSÕES: A osteotomia lateral não alterou de maneira significativa a conformação da bulla tympanica. A regeneração total da bulla tympanica geralmente não ocorreu antes de 16 semanas de pós-operatório.
